The Kumanu Care Plan is a form that runs inside a Practice Management System (PMS) that is intended to be used by GPs and nurses to create and maintain a care plan for a patient.
It is tightly integrated into the PMS and can also be integrated into Patient Dashboard to increase the visibility of the Care Plan.
Kumanu is highly configurable. Each PHO uses Kumanu in a slightly different way, with a different selection of tabs and customised terminology. The screens shown in these articles are a sample only.
One of the key features of Kumanu is Medication Management. It can be used in conjunction with pharmacist specialists to review and annotate the medications being prescribed for a patient.
The completed plan can be saved as a PDF document and emailed to the patient or attached to a referral.
Kumanu is comprised of a number of "Tabs". The first tab is where the GP and nurse defined the core information:

The exact tabs that are included in Kumanu, what they are called, the name of the fields on each screen can be configured as required. The example above is just one implementation of many.
Nearly every field is optional, so the GP and nurse only need to complete as much of Kumanu as is appropriate for the patient and the context.
