This error occurs when a form intended to be used by Medtech32 has been imported into the Evolution practice management system.
The problem is easily resolved.
- You must be logged with full practice admin rights.
- Setup → Options → Advanced Forms → Concept Map.
- Locate concept "1.3 - Location Code" and double-click on it to open it.
- Change the "Map's to Table" settings so they are as shown below.
Then logout and log in again and the problem should be resolved (for all users, but they will need to logout and in again to get the updated setting).
