Overview

The CPCT Care Plan is a form that runs inside your Practice Management System (PMS) that is intended to be used by clinicians within the practice to create and maintain a care plan for a patient.  It is supported in Medtech32, Evolution, MyPractice and Indici.

For Medtech32 and Evolution practices, it is integrated into Patient Dashboard to increase the visibility of the Care Plan.

The completed plan can be saved as a PDF document and emailed to the patient or attached to a referral.

The Care Plan is comprised of a number of "Tabs".  The first tab is where the GP and nurse defined the core information:

Nearly every field is optional, so the you only need to complete as much of the Care Plan as is appropriate for the patient and the context.

Tick this box to record that you have provided the Care Plan to the patient today.

Click either of these buttons to save the completed Care Plan as a PDF document and optionally email it to any email address.
