You can move the bar on each slider to indicate the correct value for each option. There is also a link at the top where you can download a printable version if you are wanting the patient to complete this themselves.
The values are stored in the Care Plan. When you next open the Care Plan for this patient it will be pre-populated with the previous values and you can make adjustments as required.,
